The Employee Termination Form allows managers to securely submit confirmation of an employee’s termination to Applied Business Solution. Please note that this form may vary based on your location and your form may differ from the one demonstrated in the video above.
Please verify your information at the top of the page by including the business name, your name and email address so that if necessary, we may contact you regarding the termination.
Please provide us with the employee’s name, the type of termination (voluntary or involuntary), the reason for termination as well as any additional details. Collecting and documenting as much information as possible is important when handling unemployment claims.
Please provide the last date worked as well as select the appropriate answers regarding prior counseling, equipment return and any severance granted. If there are to be severance payouts, please include the amount below. Be sure to include any additional notes that may apply to the termination at the bottom of the page as well as confirm if the employee is eligible for rehire.
